92 Abstract: Social protection of families experiencing the death of their child before and after birth The thesis analyses the issue of social protection of families who are facing the loss of their infant. This includes stillborn children, children who died immediately or shortly after birth, fetuses after miscarriage and after artificial termination of pregnancy. The thesis is divided into three main parts. The first chapter places the whole issue in the context of the present time. It discusses the terminology closely related to the topic, including its development over time. At the same time, this part also mentions the directions taken by foreign legislation and, last but not least, the scope of the cases analysed is defined. The second part, the core of the text, contains a detailed analysis of the applicable institutes that can be applied in the framework of social security law in the defined cases. Taking inspiration from the social system of the Czech Republic itself, this chapter is also divided according to the first and second pillar: into social insurance benefits and state social support benefits. Bereaved parents perception of perinatal loss

The topic of the thesis is the perception of perinatal loss by bereaved parents. Perinatal loss is a tragic event in the life of parents that is still taboo in our society and parents are not given enough support. Therefore, the aim of this paper is to describe the perception of perinatal loss through the eyes of bereaved parents, and to further outline the main needs, communication with the spouse, their family and immediate family, and support services used, with the intention of contributing to a greater understanding and awareness to guide support in the health and social care system. Palliative care in neonatology from a nursing perspective

The aim of the research diploma thesis was to find out about the system of children's palliative care with a focus on the neonatal period and to compare the opinions of health professionals on the system of palliative care in neonatology. The aim of the research part was also to evaluate the readiness of nursing professions to cooperate with the public in the field of care for premature newborns at the end of life. The theoretical part presents current knowledge in the field of perinatal palliative care. Furthermore, the theoretical part maps the current knowledge about the possibilities of support and further education of health professionals and defines the possibilities of saying goodbye to the child after perinatal loss. The empirical part presents the results of a qualitative research survey using standardized in-depth individual interviews with neonatal nurses in five perinatology centers in the Czech Republic.
Prenatal and perinatal loss entangled in Czech institutions

PRENATAL AND PERINATAL LOSS ENTANGLED IN CZECH INSTITUTIONS Marie Hintnausová ABSTRACT An ethnographic research among women who experienced a loss of child during pregnancy or postpartum aims to expose contexts and backgrounds in which prenatal and perinatal loss emerges in the Czech Republic. This life event is delineated not only by unique biographies of affected mothers, but also by the societal understanding of prenatal life and institutional definitions of human reproduction. This thesis highlights various notions and meanings entangled in the event of prenatal and perinatal death and shows which trajectories women follow in the terrain of societal expectations and governmental and biomedical institutions when they lose a promised assurance of raising a new child.
Subjective experience of perinatal loss by midwives

This Bachelor thesis deal with the topic of subjective experiencing of the loss during the delivery from the sight of midwifes. The main goal is to identify the basic spectrum of emotions and feelings related to the problematics. There are established other partial goals and research questions, which cover factors that influence providing care, strategies to handle stressful situations, psychological hygiene of the midwifes, the importence of supervision and its implementation to the praxis, burn-out syndrome in helping professions and personal experiences with this fenomen. Furthermore, there are created suggestions for practice, which alert on absence of attetion to the medical staff and comprise options, how to work in area of this problematic. Interviews in qualitative research, providing us elementary idea about problematic of the reliving attached to care of a woman during perinatal loss. Results show us that even that this process is really mentally tough, midwifes don't look for professional help. They try to solve stress situations on they own reflexion or with help from their community. Perinatal loss from the perspective of midwives

This bachelor's thesis deals with a perinatal loss from the view of a midwife. The perinatal loss is defined as an antenatal death of fetus between 22nd week of pregnancy until child delivery. This thesis is focused on the care of a midwife for a woman with a dead fetus from the time of the diagnosis to the delivery, during the puerperium and then in outpatient care. The goal of the thesis was to find out the care for woman with perinatal loss by midwives and the possibility for mothers to say goodbye after the birth of a dead fetus in selected Czech maternity hospitals. The research was conducted based on a qualitative analysis with using of semi-structured interviews. The research sample was represented by nine of midwives from six selected Czech maternity hospitals. The interviews were recorded, analyzed via the pencil-paper-method with permission of the respondents. The results were divided into two categories and related subcategories. The first research question dealt with how midwives care for a woman with perinatal loss. The research shows that midwife care is not neglected in selected maternity hospitals. The midwives, within their possibilities and knowledge, are trying to make support for women in this mentally difficult situation and to minimize trauma from loss of her child. The second research question dealt with how women can say goodbye after stillbirth in selected Czech maternity hospitals. In all facilities, parents and other family members are allowed to say goodbye to their child, cradle, care, make a photo or video. Making of memories materials is not standard in all maternity hospitals. In order to standardize the care for a woman with perinatal loss and become quality and accessible care for all concerned, there is a need to develop a unified concept of procedures and ensure the training of a sufficient number of midwives in this area. An information brochure for midwives is the output of the theoretical part of this thesis. This brochure contains elementary points of care, which midwife should not leave out during the care for woman with dead fetus. Based on the results of the research, it would be appropriate to implement a seminar for midwives. The goal of the seminar would not be only providing of expert knowledge, but also to inspire of midwives for self-reflection, to open a discussion about personal experiences and feels of midwives during the care of parents with prenatal loss. Psychosocial assistance to parents in perinatal loss

This work deals with possibilities of psychosocial assistance to parents in perinatal loss. The introduction is a short treatise on the subject of death, followed by the topic of stillborn children regarded from the viewpoint of of human dignity, and an insight into the theory of social work and the role of the (psycho) social worker. The problem of burnout syndrome in the helping professions is also mentioned. Statistics in terms of perinatal deaths from the period between 2013 to 2015 are briefly presented as well as definitions or explanations of certain terms used in the work. The subsequent chapters describe the situation of mothers before and after finding out about the child's death. It is followed by a detailed introduction of criteria for the research conducted both by the qualitative and quantitative form and is supplemented by casuistry. Everything is summarised and put into context in the ensuing discussion, and more generally summarised in the conclusion of the whole work which demonstrated the need for expert-recommended possibilities of psychosocial assistance to parents in perinatal loss (naming the child, physical contact of parents with the child, getting of objects commemorating the child, funeral - farewell to the child, information about the burial site of the child). The work also pointed out the appropriateness of including a psychosocial worker in the multidisciplinary team in healthcare facilities. The contribution of this work is the possibility of its practical use as an aid in healthcare facilities where perinatal deaths occur.
Psychosocial intervention for women with perinatal loss

This dissertation is focused on mapping the needs and development of experiencing the prenatal loss integration process in mothers, whose child died during the second or the third pregnancy trimester or suffered stillbirth. The goal of this dissertation is to suggest optimal intervention, which would prevent the progress of complications in terms of psychological disorders (PTSD, PAS). The theoretical part describes prenatal loss from the aspect of incidence and etiology and the loss of a close relative in general. It also focuses on the specifics of prenatal loss, the topic of crisis and its experience, phases of crisis, definition of the term "psychosocial intervention" and demarcation of its existing forms. This part also offers several perspectives and attitudes to the topic of grief as a process of hard life experience integration. It also describes several phases and problems connected with their fulfillment. The empirical part focuses on surveying the experiences and needs of women, who have suffered with prenatal loss. It also follows the approaches and circumstances, means of support, which helped women to cope with such an experience. Midwifery psycho-socio-spiritual care for a woman after perinatal loss

The dissertation discusses theis sues of psycho-socio-spiritual care of midwives for a woman after perinatal loss and is mainly focused on interventions that help building an attitude towards perinatal loss (farewell rituals with a baby, collection of mementos) and their influence on grieving process of women after perinatal loss. The objective of the empirical part of the study was to thoroughly describe the experiences of Czech women with parting with a child after perinatal death and investigate the impact of this experience on the grieving process after the perinatal loss. Thesub-objective of the thesis was to validate the Czech version of the Perinatal Grief Scale. We used a mixed methods research design to fulfill the dissertation's objectives. The analyses of qualitative and quantitative data were performed separately and integrated during the interpretation of research findings. The study sample comprised of total 102 women who experienced perinatal loss. The detailed description of experience of Czech women with interventions related to parting with the deceased child was dominated by the decision making process of women about farewell rituals (seeing and holding the baby, receive mementos of it). The key factors that influenced the decision making process were classified as internal (the need of the woman to get to know her child, doubt and fear of contact) and external (emotional support, information provided and a relationship with the midwife/ attending doctor) during our research. The research proved a significant influence of the independent decision of the woman about the contact with her deceased child on the intensity of grief. Grief of women who decided on their own and were confident about their decision was statistically less severe than grief of women whose decision was made by the medical personnel instead. We were checking the intensity of grief of women after perinatal loss using the newly validated Perinatal Grief Scale instrument that can be used as a single factor scale in the Czech Republic. Reliability of the research instrument was established using Cronbach alpha coefficient (? = 0.9545) and indicated high reliability. The outcomes of the mixed research point towards necessary changes in the psycho-social care of midwives for women after perinatal loss.
The potential and benefits of pastoral care in perinatal child loss

The thesis focuses on the potential and benefits of pastoral care in perinatal child loss. Discussion of the main topic of the work is preceded by a definition of perinatal child loss, information on Dead Foetus Syndrome, an introduction to new-born palliative care at neonatal intensive and resuscitative care units, as well as a reference to the experience parents go through in the death of their own child. A brief introduction of the field of practical theology is followed by the characteristics of clinical pastoral care and the potentials for its practical application. Emphasis is placed on the fact that clinical pastoral care can benefit not only sick patients and their loved ones, but also healthcare workers who are often confronted with pain and suffering during the course of their work. This discussion is then followed by a brief mention of the three theological virtues (love, hope, and faith) that are an essential prerequisite to the efforts of pastoral workers. The topic is concluded with a chapter touching on eschatological perspectives, the importance of baptism, and hope for the salvation of children who pass away before having been baptised.
